Notification period

Within 7 days of when you learned of the death.

What to bring
  • The death certificate as issued by the doctor
  • Your parsonal stamp (seal)
Applicant

This should be a family member of the same household or a person who shared a household with the deceased.
(The name of the applicant will be recorded on the family registry.)

Notification can be made to

You may notify either the municipal office of the area where the death took place, that of the permanent domicile of the deceased or the area where the applicant resides.

Notification can be made

24-hours a day

Beyond regular counter hours and on days that the office is closed, notification is accepted by the night-duty staff.

Outside regular hours, only the notification of death can be accepted. To carry out other procedures related to the death (national health insurance, nursing insurance, etc.), we request that you come to the office during regular business hours.

For matters relating to the usage of the funeral hall (SHIZUKA-NO-SATO), please contact the Environment and Conservation Division.
