Notification of Birth
Notification period
Within 14 days of the birth.
What to bring
- A notification form with a birth certificate signed by a doctor. This form is available at the hospital where your baby was born.
- Your parsonal stamp (seal)
- Mother and Child Handbook.

Either the father or mother of the baby.
Notification can be made at
Notification may be carried out at any of the followig: the municipal office of the parents' permanent domicile, that of the area where the birth took place or that of the area where the applicant is a resident.
Notification can be made
24-hours a day
Beyond regular counter hours and on days that the office is closed, notification is accepted by the night-duty staff.
Outside regular hours, only the notification of birth can be accepted. To carry out other procedures related to the birth (Proof for your mother and child Handbook, infont medical care application for child allowance, enrolling in national health insurance, etc), we request that you come to the office during regular business hours.
