Notification of pregnancy and Mother and Child Handbook issuance

To receive a Mother and Child Handbook, please submit a notification of pregnancy to the Health and Welfare Division (if you are a resident in Miki-Town).
The form required for the notification of pregnancy is available in the Health and Welfare Division. (a form obtained at a medical institution also can be used). You are not required to bring any further special documentation.
The applicant should be the expectant mother or one of her family members or friends (a person who knows the expected date of birth and the name of the medical institution where the expectant mother is going, and knows if she has had a blood test or not).
